Advise your broker that you have reported the loss to York Claim Services, Inc. Have your broker send original RHA Loss Reporting Form to Mr. Bianchi at York Claim Services, Inc., with copies to RHA.

4 Follow instructions from York Claim Services, Inc., and await contact by local adjuster.

Print out the Property Loss Reporting Form.
